Rates:
- In Network with Aetna, Meratin, Oxford, UMR, United Healthcare, UnitedHealthcare Live & Work Well EAP, Not in network with market place, medicare or medicade plans
- Minecraft: Insurance only (Aetna, Cigna, Carelon, United Healthcare, UHC EAP)
- Out of Network Cost: $150
- Coaching: $150-200 per week (insurance will not cover coaching)
